Requesting New Sick Notes

You must self certify for the first 7 days of your sickness. Many employers have their own self-certification forms. If your employer doesn't have its own form, Vist the here to complete the HMRC statement of sickness form. Please print the form, complete it and hand it in to your employer. You do not need to see a Doctor.

Book a GP appointment for a doctor’s note (also known as a “fit note” or “sick note”) for absences longer than seven days.

Please note, any requests are at the discretion of the doctor.

Requesting a Contination of a Sick Note

When requesting a continaution/ repeat sick note, Please specify how long you require. Also please state the original condition on the sick note.
