REGISTERING AS A PATIENT
To register, a patient must live in the practice area please see the catchment area to see if you can register.
The practice will refuse an application to join its list if you do not reside in our area.
We will not refuse registration because of: race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.
If you request to register with the practice you will have a new patient health check. Until you have had this you are not registed with the practice.

Accountable GP
For Patients over the age 75 a GP is assigned to them who takes responsibility for thier care.
The aims of an accountable GP are:

Continuity of Care: Patients have a designated GP who knows their medical history. They can then provide personalised care.

Holistic Approach: The GP considers all aspects of a patient’s health. Including physical, mental, and social factors.

Coordination: The GP coordinates with other healthcare providers and specialists. This is to ensure comprehensive care.

Patient-Centered: The focus is on building a strong doctor-patient relationship. This respects patient’s preferences and values.

Preventive Care: Emphasis on preventive measures and early intervention. This is to maintain health and prevent illness.

Accountability: The GP is accountable for the quality and outcomes of care provided. This often involving regular reviews and feedback.
In practice all the GPs take shared responsibility of patient care so it is not always necessary. But, if you are being booked in for an appointment you can request your named GP. We will always do our best to accomodate these requests.
